Tips for success
For the best flavor, use freshly grated Parmesan rather than pre-shredded. Freshly grated cheese melts more smoothly and has a more pronounced, nutty taste. Similarly, if you have time, make your own garlic cream sauce—it's simple to whip up with butter, garlic, heavy cream, and Parmesan.
This small effort elevates the entire dish. If using store-bought, choose one that is thick and flavorful, and don't be afraid to add a minced garlic clove or a pinch of nutmeg to enhance it.
This recipe is incredibly versatile. You can add a handful of fresh spinach or kale during the last 30 minutes of cooking for a pop of green. Sliced mushrooms, bell peppers, or onions would also be delicious additions.
If you like a bit of heat, add crushed red pepper flakes or a drizzle of hot sauce before serving. And for a crunchy topping, sprinkle some toasted breadcrumbs or additional Parmesan over each serving. The possibilities are endless—make it your own!
Variations
- Spicy Sausage Version: Use hot Italian sausage and add 1/2 teaspoon of red pepper flakes along with the sauce. For extra heat, add a few slices of jalapeño or a drizzle of sriracha before serving.
- Vegetable Add-In: Stir in 2 cups of baby spinach or chopped kale during the last 30 minutes of cooking. Sliced mushrooms or bell peppers can be added at the beginning for extra bulk and nutrition.
- Cheese Swap: Replace the Italian four-cheese blend with shredded mozzarella and provolone. Add a dollop of ricotta cheese on top before serving for a creamy, indulgent twist.
- Gluten-Free Option: Use gluten-free cavatappi or penne pasta and ensure your garlic cream sauce is gluten-free. Check that all other ingredients, including the sausage and pepperoni, are certified gluten-free.
Storage and make-ahead
- Fridge
- Store in an airtight container in the refrigerator for up to 4 days.
- Freezer
- Freeze in a freezer-safe container for up to 3 months. Thaw overnight in the fridge before reheating.
- Reheat
- Reheat in the microwave or on the stovetop, adding a splash of milk or broth to restore creaminess.
Store any leftovers in an airtight container in the refrigerator for up to 4 days. The pasta will absorb some of the sauce, so when reheating, add a splash of milk or chicken broth to bring back the creamy consistency. Reheat in the microwave or in a saucepan over medium-low heat, stirring occasionally, until heated through.
You can assemble the entire dish in the slow cooker insert the night before, cover, and refrigerate. In the morning, simply place the insert into the slow cooker and cook on low as directed.
You may need to add 30 minutes to the cooking time since the ingredients start cold. Alternatively, cook the dish fully, let it cool, and refrigerate. Reheat individual portions in the microwave or on the stovetop.
Serving ideas
Serve this cavatappi as a main course with a simple green salad and crusty bread to soak up the extra sauce. For a more complete meal, pair it with roasted vegetables or a side of garlic bread. The creamy pasta is rich, so it pairs well with a light, crisp salad dressed with a vinaigrette to cut through the richness.
Garnish each serving with a sprinkle of fresh parsley or basil for a touch of color and freshness. You can also add a few extra shavings of Parmesan on top.
This dish is hearty enough to stand alone, but it also works as a side dish alongside grilled chicken or steak. However you serve it, it's sure to be a crowd-pleaser.

- 3 cups garlic Parmesan cream sauce
- 2 cups shredded Italian four-cheese blend

Instructions
- Place the sliced pepperoni in the bottom of the slow cooker. Add the Italian sausage (casings removed) in an even layer. Pour the cavatappi pasta over the meat.
